A comparative study to assess the Vajikarana effect of Vrishya Pupalika and Modak (Aphrodisiac Food Preparations) against placebo

Introduction: Sexual health is essential for happy marital life and healthy conception. Vajikarana (aphrodisiac) can be used as preventive and curative method in the sexual health. Various food preparations mentioned under Vajikarana (aphrodisiac) therapy for healthy and diseased. Materials and Methods: The study has been designed as open randomized comparative trial with thirty healthy married males between 25 and 45 years of age which were randomly assigned into three groups, each with ten healthy volunteers. Group A or placebo Group was treated with Amalaki churna 500 mg orally in capsule form once a day with Anupana of Sharkara yukta Dugdha along with healthy sexual lifestyle. Group B was treated with Vrishya Modak 1 Pala or 48 g orally once a day with Anupana of Sharkara yukta Dugdha along with healthy sexual lifestyle, and Group C was treated with Vrishya Pupalika 1 Pala or 48 g orally once a day with Anupana of Sharkara yukta Dugdha along with healthy sexual lifestyle respectively for 3 months. Results: The results were statistically significant on various subjective parameters. In the erectile function, orgasmic function, sexual desire, intercourse satisfaction, and overall satisfaction the greatest improvement were shown in Group B, which improved by 74.28%, followed by Group C, which improved by 36.49%, and Group A, which improved by −1.99%. Conclusion: Based on the observations and outcomes, it is concluded that the result of Vrishya Modak was found better than placebo and Vrishya Pupalika.

Ayurveda is the age-old science of life. The fundamental goal of Ayurveda is to maintain a healthy person's health and to treat a diseased person's disease. This old discipline of healing and well health is still useful and relevant in today's world. To maintain a healthy person's health, many principles are described in Ayurveda such as as Dincharya (daily routine), Ritucharya (seasonal regimen), Sadvritta (codes of conduct), and Tryoupsthambha. Among these, Ahara (food) is one of the main components to maintain the health so it has been described in Tryoupsthambha. Ahara (food) is also called Mahabhaishajya. Our ancient Acharays described Ahara (food) for both healthy and diseased person. If a person takes appropriate food then he can prevent many diseases such as nutritional disorders, lifestyle disorders, and sexual diseases. Rasayana (rejuvenative tonic) and Vajikarana (aphrodisiac) therapy have been mentioned for the maintenance of lost energy due to age and loss of Shukra and other Dhatu. Various food preparations mentioned under Vajikarana (aphrodisiac) therapy for healthy and diseased. Vajikarana (aphrodisiac) can be used as preventive and curative method in the sexual health. Sexual health is essential for happy marital life and healthy conception. Most of the classical texts of Ayurveda have covered the subject of Vajikarana (aphrodisiac) in depth. If sexual intercourse is being performed without taking Vajikarana (aphrodisiac), it may lead to loss of Shukra Dhatu which plays essential role in sexual performance and conception. This condition may provide the seed for the development of the various male sexual problems. Food preparations are easily available, easy to prepare, palatable, safe on long-term use, and consumed with food very easily in routine life. So far, no such type of comparative study in food preparations for Vajikarana (aphrodisiac) effect has been reported.
Aim and objective
This study aims to compare the Vajikarana effect of Vrishya Pupalika and Vrishya Modak against placebo.

The Institutional Ethical Committee (IEC) of the “National Institute of Ayurveda, Jaipur,” has approved the study protocol vide letter No. IEC/ACA/2019/1–99 on date May 28, 2019. The trial has been registered with CTRI. CTRI Approval: “Clinical Trial Registry of India with registration no. CTRI/2020/02/023272” (Registered on: February 12, 2020).
Informed written consent from each volunteer willing to participate was taken. The detailed history was filled up in the specially prepared pro forma.
Source of data
Healthy persons registered from the premises of NIA, Jaipur.
Healthy married males between 25 and 45 years of age were included in the study. Persons willing to signature the consent form for the clinical trial.
Females, unmarried persons, males below than 25 years of age and above 45 years of age were excluded from the study. Any systematic disease such as diabetes, hypertension, mental problems, hormonal diseases, and infectious diseases were also excluded from the study.
This was interventional open-label comparative trial which purpose to assess the Vajikarana effect of trial drug. Randomization by random number table and allocation concealment method was SNOSE.
Administration of drug
Volunteers were distributed into two therapeutic groups and one placebo group with ten volunteers in each group. The detail about administration of drug is mentioned in [Table 1].
All drugs were made by the pharmacy of National Institute of Ayurveda, Jaipur. The detail about contents of Vrishya Modak is mentioned in [Table 2] and Vrishya Pupalika is mentioned in [Table 3].,
Duration of clinical trial and follow-up study
Oral administration of medicines for 3 months and follow-up was done every 15 days.
Criteria of assessment
The “International Index of Erectile Function 15 items (IIEF-15)” were applied.
Scoring algorithm for IIEF: All items are scored in five domains as follows [Table 4].
Before and after the intervention, sperm analysis was performed on all of the healthy persons who had registered.
In the analysis following things were noted - Total sperm count in millions per ml, total motility in percentage, nonmotile sperms in %, sluggish in percentage, normal forms in percentage, abnormal forms in percentage, pus cells per high-power field (HPF), and epithelial cells per HPF.
At 5% level of significance.
Statistical Software used was Graphpad Prism 9.
Statistical test to be used for nonparametric data, Wilcoxon matched-pairs signed rank test, and for parametric data, paired t-test' was used in each group, and for intergroup comparison, ANOVA test for parametric data and for nonparametric data, Kruskal–Wallis test was used, and the results were calculated.

Group A: (Placebo): Healthy volunteers treated with Amalaki Churna capsule, all subjective and objective metrics yielded statistically nonsignificant (P > 0.05) findings.
Group B: Healthy volunteers treated with Vrishya Modak showed statistically moderately significant (P < 0.01) results regarding erectile function, orgasmic function, sexual desire, intercourse satisfaction, and overall Satisfaction. In case of all the objective parameters (total sperm count, total motility, nonmotile sperms, sluggish, normal forms, abnormal forms, pus cells, and epithelial cells), the results were nonsignificant.
Group C: Healthy volunteers treated with Vrishya Pupalika showed statistically moderately significant (P < 0.01) results regarding subjective parameters such as erectile function, orgasmic function, sexual desire, intercourse satisfaction, and overall satisfaction. In case of all the objective parameters, the results were nonsignificant.
In Dunn's multiple comparisons, Group A versus Group B erectile function, orgasmic function, sexual desire, intercourse satisfaction, and overall satisfaction showed highly significant (P < 0.001) results. Thus, Group B showed better results than Group A. Group A versus Group C showed moderately significant (P < 0.01) results in overall satisfaction while erectile function, sexual desire, intercourse satisfaction, and orgasmic function showed significant (P < 0.05) results. As a result, Group C performed better than Group A. Group B versus Group C all parameters showed nonsignificant (P > 0.05) results.
In Tukey's multiple comparisons, Group A versus Group B all parameters showed nonsignificant (P > 0.05) results except nonmotile sperms variable showed significant (P < 0.05) difference. Only Group A outperformed Group B in the nonmotile sperms variable. All parameters were nonsignificant (P > 0.05) when comparing Group, A to Group C, and Group B to Group C.

The greatest improvement was shown in Group B, which improved by 74.28%, followed by Group C, which improved by 36.49%, and Group A, which improved by −1.99%.

Shukra (semen) is depleted as a result of sexual activity, and it must be replenished by food and Vajikarana Yogas (aphrodisiac products). Food recipes as aphrodisiac has been broadly described in Ayurveda because it nourishes the reproductive fluids or Shukra after nourishing the various parts of the body such as muscle, fat, and nerves. It is best to take it after a sexual act. As a result, Vajikarana (aphrodisiac) is necessary for everybody who is about to begin or improve their sexual life. As Vajikarana (aphrodisiac) is not only domain of physical health but also social, emotional, and mental health as described by Acharya Charaka.
Probable mode of action of vrishya modak
Most of the ingredients in Vrishya Modak have Madhura Rasa (sweet in taste), Sita Virya (cold potency), Snigdha Guna (unctuousness quality), Madhura Vipaka (sweet at postdigestive effect), Vata Pittahara (subsides Vata and Pitta), and Kapha Vardhaka (increases Kapha). Hence, there is an increase in sexual function. Tikta (bitter), Kashaya (astringent), Guru (heavy), Snigdha (demulsent), Sita (cold), Vrishya (viriligenic), Brimhana (nourishing), Balya (strength promoter), and Vajikarana (aphrodisiac) are some of Savyamgupta's properties. As a result of these features, it is a Dhatu Pushtikaraka (body tissue nourishing) that reduces psychological stress and enhances sperm quality. It contains a lot of dopamine. Dopamine plays a critical role in the regulation of sexual function. Madhura (sweet), Snigdha, Guru, Sheeta, Balya, Shukrala (spermatic), Vajikarana, and Brimhana are some of Godhuma's key properties. It contains the antioxidant enzyme super oxide dismutase, which transforms free radicals and other reactive oxygen species into hydrogen peroxides and oxygen molecules. Hence, it can affect HPA axis function and sex hormone regulation, as well as reduce stress. Madhura, Guru, Snigdha, Vrishya, Shukrala, Brimhana, Tarpana (Refreshing), and Balya are the primary properties of Masa. As a result, it has the ability to boost sperm count while also regulating hormone levels. Madhura, Snigdha, Picchila (sticky), Sita, Vrishya, Balya, Ruchya (pleasing), and Santarpana (satisfying) are the primary properties of Ikshuraka. As a result, it has the ability to boost sperm production. It also improves sexual activity by increasing testosterone levels. Katu (Pungent), Madhura, Snigdha, Anusna (not hot), Tridosahara (subsides Vata, Pitta, and Kapha), Hridya (cardiotonic), Rasayana (vitalizer), and Vrishya are the primary properties of Pippali. As a result, it reduces mental tension and anxiety while also stimulating hormones that promote sperm motility, sexual potency, and fertility. When combined with other herbs, it enhances their effects (Yoga Vahi). Madhura, Guru, Snigdha, Sita, Hridya, Vrishya, and Balya are the major properties of Priyala. Due to the Balya and balanced nature of Tridosha, it relieves fatigue symptoms. Its Vrishya characteristic aids in increasing stamina and reducing sexual dysfunction. Madhura, Kasaya, Laghu (light), Sita, Prasadana (solidifying), Ropana (healing), Vrishya, Srotosodhana (clearing the channels), and Yogvahi (catalyst) are some of the properties of Kshaudra. Madhu is aphrodisiac in nature due to the qualities described above. Because it is Yogvahi, it enhances the qualities of other ingredients. Srotosodhaka has the potential to cleanse the body's pathways, allowing other medications to access the body more easily, particularly in the Shukravaha Srotasa (semen carrying channels). Madhura, Guru, Snigdha, Sita, Balya, Vrishya, Vayasthapana (anti-aging), Ojovardhaka (increases essence of all the seven Dhatus), Hridya, Rasayna, Shukravardhaka (increases semen), and Tejobalkra are the primary properties of Ghrita. These characteristics are necessary for optimum sexual health. It mostly comprises fat and protein-rich macronutrients, as well as Vitamins A, E, K, D, and C, as well as minerals such as calcium, iron, magnesium, and phosphorus. All of these nutrients aid in increasing sexual potency. Madhura, Guru, Sita, Dhatuvardhaka, Hridya, and Vrishya are the primary properties of Sitopala (Sharkara). As a result, it has the ability to replenish sperm, relieve exhaustion, and increase sexual vitality.
Probable mode of action of vrishya pupalika
Pippali, Masa, Godhuma and Ghrita are mentioned as above. Madhura, Snigdha, Sita, Hridya, Vrishya, and Brimhana are the primary properties of Shali Dhanya. As a result, it keeps people energized and increases sexual potency. Madhura, Ruksha (dry), Mridu (soft), Sita, Vrishya, Vatahara (subsides Vata), and Sthairyakara (gives stability to the body) are the primary properties of Yava. Due to these properties, it has the ability to increase genital organ circulation, sexual potency, and penile erection.,

Based on the preceding results, the alternate hypothesis is that there is a difference in Vrishya Pupalika, Vrishya Modak, and placebo in terms of Vajikarana effect. Vrishya Modak found more effective improvements in all subjective indicators than Vrishya Pupalika and placebo. No side effects or complications were reported during the study period. These medicines are found effective, cost-efficient and can be used by anyone who wants to improve their sexual life.
